제목 Cauda equina syndrome after caudal epidural injection under severe spinal stenosis

Caudal epidural injection is common treatment to treat low back pain and sciatica. Caudal epidural injection can rarely lead to neurologic complications. We describe a case of cauda equina syndrome after caudal epidural injection. A 44-year-old male patient with severe L4 and L5 spinal stenosis underwent caudal epidural injection for low back pain and sciatica. Caudal epidural injection solution consist of lidocaine, bupivacaine, hyaluronidase and triamcinolone acetonide. He experienced motor weakness and sensory loss on the both low extremitiy and urinary incontinence after the procedure. Ankle dorsiflexor, big toe extensor and ankle plantar flexor were checked motor power MRC grade 0. Ankle jerk was absent. The electrophysiologic study shows compatible with lumbosacral polyradiculopathy, affecting L5 and S1 nerve roots on the both sides. We suggest that mechanical damage which caused by solution in patient with spinal stenosis occurs cauda equina syndrome.
