제목 Computerized reaction time test for evaluation of cognitive impairment in stroke patient

Objective : Paper and pencil tests such as line crossing test, star cancellation test and line bisection test are widely used for the evaluation of hemi-neglect after stroke. However, these tests were hard to evaluate visuospatial attention on specific location or time efficiency of perception. We previously developed computerized dynamic visuospatial test and applied it to stroke patients who had hemi-neglect to evaluate visuospatial efficiency through measuring visual reaction time. But, computerized inattention test which we developed was not more accurate assessment for neglect than paper and pencil tests. However, we discovered that the more patients have impaired cognitive function, the more reaction time prolonged in this computerized test. The aim of this study was to identify the correlation between cognitive function and reaction time by using the computerized dynamic visuospatial test.

Method : 28 stroke patients were recruited from Dec 2013 to June 2014. We made computer program which display a quadrangle at random location on 50 by 28 centimeter monitor divided by 8 x 4 space and explained the examinee to press the key-board as soon as they saw a quadrangle. In this manner, we measured visual reaction time for each location and identified unresponsive space if examinee didn't press a key-board more than 5 seconds after appearance of a quadrangle. Then we calculated average reaction time and checked K-MMSE (Korean Mini-Mental State Examination) for evaluation of cognitive function.

Result : The more patients scored low grade in K-MMSE, the more patients showed prolonged reaction time in the computerized dynamic visuospatial test. (p-value < 0.05)

Conclusion : We found out that reaction time have closely related to cognitive impairment in stroke patient and the computerized reaction time test can be used substitutingly to evaluate overall cognitive deficit.
